the first is a bit more polite. the speaker is asking for help in a off hand way. The second is expecting the other to help. The second is not rude. but less formal. you would use it among friends.
I can get through the ceiling if you give me a hand.

the first sentence. " I can get through the ceiling if you give me hand " is like your telling that you'll be able to get through with his help
the second one is like your asking for help. ^^

" I want to get through the ceiling, can you give me a hand ?" ^^
