Both, but it would sounds more natural, if you switch the position of "das Buch" and "dem Arzt"
Ich habe dem Arzt das Buch gegeben.
Ich gab dem Arzt das Buch.

I think it's important to know that there's a trend in Germany to only use the Perfekt version with "haben" and "forgetting" the Präteritum. I read that there are some places in Germany where the Präteritum isn't used at all anymore and many people, especially younger ones, would find the (extensive) use of the Präteritum to be archaic.
