The first sentence is in the present tense and kinda states a fact. The second sentence is a bit more of a hypothetical one. To be honest, both sentences don't really make that much sense to me, but that's the only difference I can tell right now.
I think a better version of both sentences would look like this:
1) Live as if you are dying.
2) Live as if you were dying.
Replace the "like" with "as if". Then you would be trying to express a scenario, instead of telling someone to live like they are already dying.
