"Can I help you?" sounds a bit more rude when you're speaking to a customer.
In certain situations, you can use "Can I help you?" to simply ask if you want to help someone.
For example:
A: "Hey, what are you doing?"
B: "I'm trying to make a paper airplane."
A: "Oh, cool! Can I help you?"
B: "Sure!"
"How can I help you?" or "How may I help you?" sounds nicer and should be used when speaking to a customer. You may also ask, "Do you need any help?" ^^

"Can I help you?" is usually used if the speaker believes the listener needs help, but need confirmation from the listener to see if this is true. For example:
At a shop:
"Can I help you?"
"No, I'm okay. I'm just browsing."
"Can I help you?"
"Yes, I was wondering if you have these shoes in my size."
"How can I help you?" is used when the listener has already confirmed that help is needed. For example:
*Listener calls a phone number for a helpline*
"Hi, thanks for calling our helpline. How can I help you today?"
"I would like help with..."
*Listener walks up to the counter at a dentist clinic*
"Hi, how can I help you?"
"Hello, I'm here for my appointment. My name is..."
