I found the example sentence below in a dictionary.
‘She smiled slowly as horror registered on his face.’
I couldn't get which happened first, she smiled or he showed fear.
Could someone tell me which was first, please?
Also, if 'as' was 'when' in the example, does the meaning of the sentence change?

They both happened at the same time. Using when is slightly unnatural with "slowly" as the latter implies something which took a time to happen and "when" implies here a fixed time point. You could however use "while" to means the same as "as".
If you say "She smiled when horror..." then the horror comes first.
