So, say my boyfriend is practicing piano and I walk in the room. He gets distracted because I opened the door and he plays a wrong note and loses his place in the music. I notice this, wave awkwardly, and leave him be so he can continue. Later, during supper, I refer back to what happened, saying "I'm sorry to have disturbed you during your practice." And, hopefully, it wouldn't be a big deal to him and we'd move on.
In this case, I'm referring to the disturbing that I did in the past.
Now, in another scenario, say he is practicing piano, but supper is ready and I would like to let him know, so that he can come eat if he wants to. So, I come into the room and say "I'm sorry to disturb you, but supper's ready if you'd like to come eat."
In this scenario, I'm referring to the disturbing that I'm in the process of doing (even though I have a good reason).

“I’m sorry to disturb you” is usually said before you ask someone for help on the street/in a store/etc.
“I’m sorry to disturb you, but can you help me find the bus stop?”
“I’m sorry to have disturbed you” is usually said after you end the conversation.
“Thank you for your help! Sorry to have disturbed you.”
