I heard that "Couldn't be better." is a very common phrase as a reply to "how are you doing?"
But "Couldn't be better." is "絶好調です。" in Japanese, so it seems to me as if someone felt by far the better.
Is it ok to say "Couldn't be better" when I feel as good as usual?

I would be grateful if you corrected any mistakes in my English.

Yes, it is alright to say "Couldn't be better" when you are asked how you're doing. Usually in English when you are asked "How are you?" you are expected to day that you are doing well, which is what "Couldn't be better" means.

I don't think it's very common, I've heard more of "same as always". When people ask "What's up?", the most common neutral answer is "Not much." c:

depending on how casual you are talking, say to a friend, you can say "same as always" or just a simple reply of "Good" is a nice answer.

saying that, kinda makes you sound a little more confident.

it is not a formal answer tho, but it is still acceptable

"Couldn't be better" is acceptable, but a more common answer to "How are you doing?" is "I'm fine" or "I'm doing good."

"What are you doing?" and "What's up?" are pretty similar. "What's up?" is asking more about what's happening recently in life, generally. "What are you doing?" is more of asking what plans you have for that day. :)

@komori: "what are you doing" is used when literally want to know what someone is doing while "what's up" is more casual and can mean a lot of different things. For example they could be asking how you are doing or what you need (usually after you telling them that you need to talk)

"couldn't be better" is also commonly used in a sarcastic way when all you could be is better

@komori: it is relatively dry. I think it depends on how you say it and who you say it to. for instance, if I say it in a high pitched voice and with excitement, it won't sound as dry and rude.
