を is for direct object
に is for indirect object
は is for subject. for simple description.
が is for subject. が is used with the answer of the question that is assumed in the context.
ex) For introducing yourself, you say 「わたしは山田です。」Totally correct. If you say,「わたしが山田です」Then people think, "Eh, is this YAMADA guy a kind of celebrity or has everybody heard rumors of him?", because "が" is supposed to be used with the answer of the question. If people don't know the question it sounds a little weird. Then people try to guess, he might be a popular man is an easy one. If this YAMADA guy is a really scary man and people heard of it as a rumor or something, 「わたしが山田です」 makes sense. People think "oh, he is! what the hell is gonna happen to us!"
Another example
Someone asks "Who will go?"
. Correct. It is the answer of the question of this context, and a answer subject is displayed with "が".
Correct, since this subject is not the answer of the question.
If you say, 「わたしは行く」this means "I will go whatever. I don't care who goes or not" because this subject can be an answer but still use "は" it shows you don't answer the question. You say it as a new story or braking the rule, and it shows you don't care this context. People can think you are an egoist. As far as you are gorgeous it doesn't matter.

Talking about interrogative sentences, when you ask a description of something, you should use "は". When you ask to choose answers, you should use "が".
For instance:
What is your name? (Asking a description) My name is Yamada.→
Which is yours?(Asking to choose). This is my one.→
Who are you? (A description) I am Yamada. →
Who is Yamada?(Choosable) It's me.→ 誰が山田ですか?
What is the truth? (Choosable ) This is the truth. →
What is the truth? (A description ) The truth is this.→ 真実は何なのか?
