It really depends on the person you're talking to. However, I do think that people from Portugal understand Brazilian people better (maybe it has to do with how exposed we are to Brazilian Portuguese because of their telenovelas, but I'm not sure).

As a Portuguese person, I think it's pretty easy to understand Brazilian Portuguese, though there can be sometimes where they use words I don't know or the accent is too strong. In television they normally don't even use subtitles in the news and stuff.
Still that can happen even while speaking Portugal Portuguese because of the different accents for example from Açores or North Portugal.
In general I think there aren't much understanding issues.

Yes Brazilian Portuguese is very simple when comparing to Portuguese from Portugal. They open more their mouth to say the words so it's easier to understand. People that are learning Portuguese from Portugal normally say they find themselves having a hard time learning or understanding this when comparing to Brazilian Portuguese.

It is easier for a Portuguese person to understand Brazilian portuguese , than the Brazilian understand the Portuguese from Portugal. I have met a few brazilians over my life , and the friends I made they took at least one month to get used to my accent and understand what I am saying. While I always understood what they say when they talk.
