My dictionary says:
1. “Dear John Smith” is not commonly used.
2. “Dear Mr. Smith” or “Dear John” is OK.
Then, how about “Dear Mr. John Smith”?
Is this not so common (but possible or passable), or very strange?

Hahaha, that's a nice hybrid, if you will.
Yes, I think it can be used, but it is not the standardly used form when addressing someone in a letter. I'd say it sounds more formal.😄

@iamaimo Yes, it's good, you can use it when you write a letter. "Dear Mr. Smith" is more formal and often used, but, in my own opinion, you can use use "Dear Mr. John Smith."

All three are fine, using Mr. makes it more formal. You could write "Dear + first name" if it is an informal letter.
Also a "Dear John letter" is famous idiom that means a letter used to break off a relationship with a lover.

@iamaimo if its a professional letter you should use Mr/Mrs or their title and last name only, like Dear Professor Smith or Dear Mrs Smith. An important point though is to only use a person's title if you would using it when addressing them in person. So if you write a letter to your manager you wouldn't say, Dear Manager Smith for example. Using both first and last names feels very formal and impersonal. You might see it in a a bill you get in the mail or a notice from a company, like your cell phone provider. "Dear John" feels very casual and more like a letter between friends.
