"I'm worried about ____" = Something that is bothering you today, right now, and is usually directly connected to your life.
"I'm worried about my son's grades in school. I'm worried about my uncle. I'm worried about what will happen if I get sick."
"I worry about _____" is usually used more with big issues that might be problems in the future, or things in your life that are not a problem now but you think will be in the future.
"I worry about what will happen when my son goes to college. He has never been responsible."
"I worry about retirement. I worry about the war in the Middle East."

I think the first is in passive voice and the second is in active voice.
Other than that, they are saying the same thing.
